• Court-mandated therapy

• Crisis response/24 hour services

• Individuals who have active suicidal or homicidal intents/plans

A 15- minute phone call where we discuss your initial concerns/why you are seeking therapy, answer any questions you may have, and assess if my skillset/areas of specialty are a good fit for your needs.

If it is determined that we are not a good fit to work together, I would be happy to provide you with referrals to other service providers.

If we both agree to work together, you will be sent paperwork to fill out and an intake will be scheduled.

Intake appointment is where we will have more time to discuss your experiences and set up a treatment plan.

No, Sculpted Phoenix Psychology LLC is an out-of-network provider.

Yes! As long as your insurance policy has out-of-network coverage for mental health care.

Start by calling your insurance company via the information provided on your insurance card.

Ask, 'Does my coverage include out-of-network benefits for mental health care?' and 'Specifically, for outpatient psychotherapy via tele-health?'

If your insurance plan provides out-of-network coverage for mental health care/outpatient psychotherapy via tele-health, see, “How much will services cost with insurance reimbursement?” & “How to request a superbill?” for more information.

This will depend on your insurance plans deductible (if you have one) and the treatment code utilized for services.

Ask, 'Do I have a deductible that I have to meet before out-of-network tele-health services would be covered?' and 'If so, what amount is remaining before I am entitled for reimbursement for any associated out-of-pocket fees?'

Service codes are subject to change based on client need/service provided. A good place to start is by asking, 'How much does my plan cover for mental health service code 90837 when seen by a Psychologist?'

A superbill can be generated monthly by the provider, send to you via secure portal message for you to access and submit to your insurance company, if you wish to do so.

My standard hourly rate is $300.
